The first step to transferring to our office is to complete a medical record release so that we can retrieve your records from your previous doctor. The practice functions most effectively as a medical home if patients/families provide a complete medical history and information about care obtained outside the practice. Please keep in mind that your previous office has up to 30 days to send the requested records to our office.
